The Health Risk Assessment Wellness Coach conducts health risk assessment results sessions. Assists team members and/or spouses in identifying their health risks, interpreting results, and facilitating personal wellness plans. Is responsible for the delivery of patient care that meets the high quality of care provided by ThedaCare through collaborative practice with all members of the care team. Contributes to and is accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development. Reviews health risk assessment results to help clients understand lifestyle-related health risks, assess readiness for change, set health-related goals, and develop and implement effective strategies to reach those goals. Provides education on how clients can improve lifestyle behaviors through knowledge of how lifestyle behaviors impact and affect health risks. Collects a complete and accurate participant list, including no-shows, walk-ins, etc., for billing purposes. Collaborates effectively with internal and external clients to promote and support wellness initiatives and facilitates a positive environment for wellness. Interprets, evaluates, and communicates health risk assessment data for clients. Facilitates the on-site health risk assessment session to ensure efficient customer flow and timeliness of appointment to provide quality individual service. Follows ThedaCare safety standards and policies along with following non-ThedaCare employer group safety policies as we move through their businesses. Qualifications include High School diploma or GED preferred, must be 18 years of age, American Heart Association Basic Life Support (BLS) or CPR certification for Healthcare. Physical demands include ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to 25 pounds without assistance. Job classification is exposed to blood borne pathogens while performing job duties. Work environment is primarily climate controlled office setting with daily movement throughout the campus and occasional exposure to various hazards. Scheduled weekly hours are 20 with a 0.5 FTE. Location is ThedaCare at Work - Capitol - Appleton, Wisconsin.
