Seeking Social Worker to join a team that provides compassionate care to the frailest elderly who have chronic and acute illnesses living in their homes. Our LISW delivers care and counseling to a wide variety of patients with social and behavioral problems. The Social Worker will be accountable for caring for patients, connecting patients and their families to support services, maintaining accurate and current patient records and scheduling and administering initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our Physicians, Advanced Practice Clinicians, and Care Team Coordinators.

Responsibilities:
Community Social Worker (MSW) Responsibilities:
Conduct psychosocial screenings with patients in person or via telehealth to identify social determinants of health and support needs.
Coordinate access to essential community resources and services based on patient needs.
Collaborate with Behavioral Health Specialists and the care team to support patient-centered care plans.
Participate in interdisciplinary team meetings to align on patient progress and care goals.
Document key interactions and care planning activities.
Maintain current written progress notes and other documentation on the member Medical Record including signed and dated documentation for all service in compliance with WellBe documentation policy.
Effectively collaborates with the family, primary caregivers, and appropriate community agencies to meet the patients’ care goals.
Collaborate and align with the community team on quality and performance measures and improving patient outcomes.

Community Social Worker (MSW) Qualifications:
Master’s degree in social work
Minimum of 2 years post-Masters clinical social work experience within the behavioral health field.
Have a valid driver’s license, automobile insurance, have access to an automobile; and proof of automobile insurance required.
